New Student Checklist

In a short time, you will join the Wright State family as a member of the Class of 2013. Please reserve the day of August 2 for Convocation and the White Coat Ceremony, an event that you will want to share with your family and friends.
As we complete our checklists, we ask that you check to see that you have completed yours. Here are some requirements that must be met for your enrollment completion. Please read this information carefully and note the dates that information is due back in our office. You may download a copy of this check list suitable for printing.
Have you done the following?
- Sent official transcripts? The state of Ohio requires that you provide us with official transcripts from all colleges and/or universities you have attended, with your degree(s) posted. The deadline is June 30. Please remember that the transcript that you release to AMCAS when applying is NOT sent to us. You do need to request transcripts to be sent to us with your degree listed by June 30. Have your transcript mailed to:

- Filled out your Personal Profile? To help you get to know your Year I classmates, the Office of Student Affairs and Admissions will prepare a Class of 2013 book for distribution during Orientation. The book will contain pictures and interesting information to help you and your fellow incoming students become better acquainted. Please sign in with your new WSU user name and password, complete an Introductory Survey and verify your biographical information. If you do not know your user name and password, please e-mail Debbie McNeely in Student Affairs and Admissions or call her at (937) 775-2934.
- Submitted your Student Health Record? This must be completed by you and a physician before you can begin classes. The deadline is July 10. You may download the form as a PDF file or a Microsoft Word document. Mail it by the deadline to:

We recommend that the following written reports accompany your Student Health Record:
- Most recent chest x-ray report
- Urinalysis
- CBC (including platelets)
- Submitted your Immunization Record? All immunizations, unless otherwise noted, must be completed in order to start classes. The deadline is July 10. You may download the form as a PDF file or a Microsoft Word document. Mail it by the deadline to:

Maintenance of health records
All medical information will be confidential and will be released only at the request of the particular student who signs a written release form. Each health record will be reviewed by a physician from Boonshoft School of Medicine and further recommendations might be made for follow-up testing or information. WSU Student Health Services will review immunization records, administer required immunizations for medical students and/or refer to the student's physician, and maintain the above medical information on file.
Immunizations are available from Student Health Services in Room 051 of the Student Union. Clinic hours are 8:30 a.m.-noon and 1-4:30 p.m., Monday-Friday. Immunizations are given 8:30-11 a.m. and 1-4 p.m. There is a small fee. Student Health Services does not give immunizations to pregnant students. |
