  • Copy of Current Curriculum Vitae (CV)
  • Passport Sized Photograph
  • Copy of Medical School Degree
  • Copies of United States Medical Licensing Examinations (USMLE):
    • Step 1
    • Step 2 Clinical Knowledge (CK)
    • Step 2 Clinical Skills (CS)
    • Step 3
  • Copy of Educational Commission for Foreign Medical Graduates (ECFMG) Certificate (If applicable)
  • Copies of all Current Medical Licenses
  • Copy of Residency Training Certificate(s) (If Applicable)
  • If Only a PGY-1 Year Has Been Completed, a Copy of the Summative Evaluation of Competencies Form (Global Resident Competency Rating Form)
  • Copy of Board Certificate(s) (If applicable)
  • Copy of Other Graduate Level Degrees and / or Aviation / Aerospace Medicine Courses (If applicable)
  • Three (3) Professional References (Reference letters / Letters of Recommendation should be on office letterhead and sent via USPS/FedEx /UPS, etc., carrier.  Emailed reference letters/letters of recommendation will not be accepted.)

International Physicians Only

Demonstrated proficiency in English is required. If the applicant's native language is not English, a minimum score of 213 (CBT) or 79/120 (IBT) on the Test of English as a Foreign Language (TOEFL) is required or a band 6 through the International English Language Testing System (IELTS). Students who have earned a degree from a regionally accredited U.S. college or university are exempted from the TOEFL requirement. For the latest information, international physicians are advised to refer to the general requirements of Wright State’s University Center for International Education (UCIE).
